The Office of Gift Planning at Northwell Health provides support for a full range of charitable gift-planning options and services. 

We strongly urge prospective donors to consult with their attorney, financial advisor, estate-planning professional, accountant, or other appropriate professional before making any material decisions based on information we provide through this web site, printed materials, or other sources.  We welcome the opportunity to work with you in helping to identify tax-wise and beneficial ways to support Northwell Health’s charitable non-profit mission.  Contact us directly for further information or assistance on a confidential basis.

Please feel free to contact the Office of Gift Planning at (516) 321-6260 if you require a copy of our IRS tax letter or for a Form W-9. You can find our suggested bequest languge here.

Estates: The administration of estates for all 16 hospitals (and many of our affiliated entities) is centralized and headed by the Northwell Health Foundation. Checks representing distributions from wills and trusts should be made out to "Northwell Health Foundation" and sent to the attention of Alexandra P. Brovey, Senior Director, at 2000 Marcus Avenue, New Hyde Park, NY 11042. Releases and other official documents will be signed by Ralph A. Nappi, President.

Please send a copy of the will or trust (or other legal instrument), and specify the date of birth and date of death of the deceased donor.

All donors of planned gifts - including wills and trusts - will be recognized as members of the Payson Wharton Legacy Society.
